HOME HEALTH AIDES: Territories Needed: Norwich, Hartford and Meriden areas RESPONSIBILITIES: Demonstrates professional growth, and competency. Accepts clinical assignments that are consistent with education and competence to care for patients. Provides necessary skills to assist the patient with safe transfers, exercise, and ambulation per agency policy. Assures continuity of quality patient care delivered according to the Aide Plan of Care with appropriate documentation. Promotes Agency philosophy and follows Agency policies, health promotion, and disease prevention REQUIREMENTS: *Active CNA certification as required by state or federal regulations *High School diploma preferred, or GED equivalent *Valid driver’s license and automobile insurance *Minimum one year experience as a Home Health Aide or Nursing Assistant in a health care facility *Current CPR certification *Must be able to lift 80-100 lbs.

Afternoon Admission RN
The Admission RN is primarily responsible for admitting new clients to the homecare program per state and federal regulations. He or she will accomplish a complete assessment of the patient using the OASIS tool, review all admission paperwork with the client, and gather all necessary patient signatures pertinent to admission. The person in this position will also assist with Resumptions of Care after patient hospital stays. Time not spent on admissions will be filled on regular visits. This position is currently being offered for full-time, which includes company benefits, such as PTO, medical, dental, vision, and 401(k). Candidates must be a current Registered Nurse in the State of Connecticut and have prior experience in homecare, especially in case management. and admissions.
